"IF YOU BUILD IT, THEY WILL COME." ALTHOUGH these words are most commonly associated with the cornfield that became a Field of Dreams, they may be heard in the hospital in reference to information systems. In this context, "It" refers to a multi-million dollar information system (IS) that, among other key functions, will allow physicians immediate, paperless access to data regarding their patients, and "they will come" means that these same physicians will promptly line up ready to use it. Unfortunately. physicians often fail to embrace a complex information system, may not see its relevance to their practices, and are characteristically reluctant to invest the time and energy to be trained in its use. This article explores some of the reasons that physicians demonstrate little "buy-in" to this process and offers suggestions to help create a more successful Implementation. Many hospitals and health care organizations have implemented new, integrated information systems that capture data upon initial patient contact and make that information available to all departments on a real-time, online basis. Typically, implementation of the new system is mandated for all business departments and acceptance can be assured because of the controls that the hospital has over Its employees. Nursing and clinical support functions are included; nurses may chart directly on bedside personal computer (PC) workstations. The system simplifies record keeping by allowing charting of variances to established clinical pathways, that is, charting "by exception." Laboratory personnel benefit from the direct, automated entry of results into the clinical system. They can focus their quality control efforts on the equipment and not on the human interface. Time spent on the telephone reporting results to the medical wards can be reduced.
